Dear faculty and staff,

Through our partnership with St. Luke’s University Health Network, the College has coordinated on-campus COVID-19 vaccination this Monday and Tuesday in Kirby Sports Center. We have prioritized students in the scheduling of single-dose Johnson & Johnson COVID-19 vaccine appointments, but we still have a few hundred appointments available, mostly on Tuesday.

While faculty and staff have been able to register for a COVID-19 vaccine under the 1A or 1B designation, we are extending appointment slots for all employees and their immediate family members who are 18 and older. (If you already have a vaccination appointment for the upcoming week, please keep it.)

To register for an appointment, you must complete a two-step process.

Important details:

  • If there are no time slots displayed in the Qualtrics survey, that means there are no appointments available.
  • Ensure that each patient has his or her own St. Luke’s MyChart account.
  • If you have technical trouble creating a MyChart login, there will be on-site help, and you should still plan to attend your vaccination appointment.
  • You can only enter one name and choose one time slot each time you complete the survey. If you choose two time slots accidentally, you will only be signed up for the earliest slot you selected.
  • You can complete the registration more than once to accommodate multiple family members.
  • For questions about MyChart accounts, please contact St. Luke’s (1-866-785-8537, option 5).
  • For other questions (after you read the webpage and fill out the survey), please email covid-19@lafayette.edu.

Sincerely,

Annette Diorio
Vice President for Campus Life
