Student flu shots will be offered at the Bailey Health Center Tues., Oct. 18, and Wed., Oct. 19 from 9-11:30 a.m. and 1-4:30 p.m.
The total cost of $20 can be billed to your school account or you may pay cash. Receipts will be given to students to submit for reimbursement to their insurance company
No appointment is necessary. Please print and fill out this consent form and bring it with you.
The vaccine being offered this year is Sanofi Pasteur Fluzone Quadrivalent.
